5 Postpartum Thrombophlebitis Nursing Care Plans

Superficial thrombophlebitis is more common after birth than during pregnancy, especially in women with varices. It hits the saphenous vein of the lower leg and shows up as a painful, hard, reddened, warm vein you can see.

About 50% of DVT patients are asymptomatic, which is what makes DVT dangerous: it propagates to PE, post-thrombotic syndrome, chronic venous insufficiency, and valve destruction. DVT runs anywhere from the feet to the femoral area, with pain, calf tenderness, leg edema, color changes, and pain on walking. A positive Homan's sign may appear but is unreliable postpartum and not specific to clots.

PE happens when a clot breaks off and lodges in the pulmonary circulation. The presentation can be dramatic: sudden chest pain, cough, dyspnea, a dropping level of consciousness, and signs of heart failure.

Thromboembolic disorders hit less than 1% of postpartum women, but PE can kill if a clot obstructs lung circulation. Early identification and treatment are everything. Nursing management prevents clots, restores circulation when one forms, and teaches the patient about anticoagulation and danger signs.

Nursing Care Plans and Management

Goals: improve tissue perfusion, resolve the thrombus, control pain, prevent complications, and give the patient information and support.

Nursing Problem Priorities

  • Anticoagulant therapy. Start and manage anticoagulation to stop clot progression.
  • Pain management. Control the discomfort of the inflamed vein.
  • Monitoring for clot progression. Watch for new swelling, redness, or warmth.
  • Infection control. Prevent or manage infection that complicates the picture.
  • Ambulation and mobility. Promote early movement to improve circulation.
  • Education and support. Teach the condition, its symptoms, and the need to stick with treatment.
  • Followup and monitoring. Track treatment response and adjust as needed.
  • Collaboration. Coordinate with vascular specialists or hematologists.
  • Prevention. Use early ambulation, hydration, and compression stockings in at-risk patients.

Nursing Goals

Goals and expected outcomes may include:

  • The client will show improved perfusion: palpable, equal peripheral pulses, good capillary refill, reduced edema and erythema.
  • The client will use behaviors that improve peripheral perfusion and tolerate increased activity.
  • The client will maintain adequate ventilation and oxygenation with ABGs in normal range and report relief of respiratory distress.
  • The client will report pain relieved or controlled and use comfort techniques.
  • The client will verbalize anxiety, report it reduced to a manageable level, and show fewer signs of restlessness and irritability.
  • The client will verbalize understanding of the condition, treatment, and restrictions, perform therapeutic actions correctly, and identify symptoms requiring medical evaluation.

Nursing Interventions and Actions

1. Preventing Thrombus Formation

Pregnancy raises venous thrombosis risk through stasis: the heavy uterus compresses vessels, and stirrups press behind the knees during repair. Varicose veins and bed rest add to the hypercoagulable state. When a clot embolizes to the lungs it causes sudden chest pain, dyspnea, and a falling level of consciousness, and a large enough clot can cause sudden death. PE is the leading cause of pregnancy-related death in the United States, occurring in 2 out of 100,000 live births.

Assess vital signs closely. Watch for a slight temperature rise, possibly to 101℉ (38.3℃), and report it. Because a DVT patient can throw a PE, report any sudden onset of breathing difficulty immediately.

Monitor capillary refill time. Diminished refill is common in DVT. A positive Homan's sign is no longer diagnostic; one study found it present in only 1/3 of subjects with confirmed thrombosis. Calf pain can come from a strained muscle or contusion. Homan's sign matters only alongside ultrasonography and venography.

Assess risk factors and signs of thrombophlebitis. Look for oral contraceptive use before pregnancy, smoking, prolonged standing, prior thrombosis or endometritis, and current varicosities. Add prolonged bed rest, diabetes, obesity, cesarean birth, severe anemia, advanced maternal age (older than 35), and multiparity.

Differentiate SVT from DVT. Localized edema, redness, warmth, and tenderness point to superficial involvement. Pallor and coolness point to DVT. Calf-vein DVT usually has no edema; mild to moderate edema suggests femoral involvement, and severe edema is characteristic of iliofemoral thrombosis.

Assess respirations and auscultate lung sounds for crackles or friction rub. Investigate chest pain or anxiety. Watch for PE: sudden shortness of breath and severe chest pain. The patient may be apprehensive and diaphoretic, with tachypnea, falling oxygen saturation, and a sudden mental-status change from hypoxemia.

Assess for pain or tenderness in the lower extremities. Suspect superficial thrombosis in a patient with varicose veins who reports tenderness over the site, usually the calf, with redness and warmth along the vein. Pain increases with weight-bearing.

Examine extremities for prominent veins. Palpate gently for tissue tension, stretched skin, and knots along the vein. Superficial vein distention can occur in DVT from backflow through communicating veins.

Instruct the client not to massage or rub the affected extremity. Massaging the clot can loosen it and cause a pulmonary or cerebral embolism.

Elevate the feet and lower legs above heart level when sitting or lying down. With bed rest and elevation for DVT, this reduces interstitial swelling and promotes venous return.

Instruct the client to avoid constrictive clothing and crossing the legs. On bed rest, change position often but avoid sharply flexed knees that pool blood in the lower legs.

Encourage fluid intake of 2500 ml/day unless contraindicated. Dehydration raises blood viscosity and venous stasis.

Emphasize deep breathing exercises. Regular deep breathing may help prevent venous thrombosis in postoperative patients.

Assist with active or passive ROM during bed rest and gradual resumption of ambulation. Short, frequent walks beat one long walk. In bed, flex, extend, and rotate the feet periodically.

Apply warm, moist compresses to the affected extremity as ordered. Check the temperature first; altered blood flow can blunt the patient's sense of temperature and risk a burn. Keep the compress weight off the leg, and protect the linens with a waterproof pad.

Apply support stockings as prescribed, avoiding a tourniquet effect. For varices, medical support stockings worn the first 2 weeks after birth improve venous circulation. Put them on before rising; waiting until the patient is up lets venous congestion set in. Remove them twice daily to check the skin for mottling or inflammation.

Apply sequential compression devices and TED stockings as indicated. Fit them correctly to avoid excess pressure. Use compression devices for varicose veins, a thrombophlebitis history, or surgical birth.

Monitor labs: AST, LDH, PT, aPTT, hemoglobin (Hb), and hematocrit (Hct). A therapeutic aPTT typically runs 35 to 45 seconds, depending on the reference values used. Hemoconcentration raises clot risk; the coagulation profile flags clotting problems.

Administer medication for postpartum thrombophlebitis as indicated. See Pharmacologic Management.

Prepare for surgical intervention as indicated. Thrombectomy is done when inflammation extends proximally or circulation is severely compromised. Recurrent thrombosis unresponsive to or contraindicated for anticoagulation may need an inferior vena cava (IVC) filter.

Monitor vital signs and cardiac rhythm. PE shows tachycardia (>100 beats/min) and tachypnea (>20 breaths/minute). A loud P2, a right-sided gallop, and increased central venous pressure point to pulmonary hypertension from PE.

Auscultate for decreased or absent breath sounds and adventitious sounds. A pleural friction rub, crackles, and decreased breath sounds signal pleural infarction.

Note respiratory rate, depth, and work of breathing. Tachypnea and dyspnea accompany pulmonary obstruction and may be the only signs of subacute PE. Severe distress means a large loss of functional lung units.

Assess level of consciousness and mentation. Hypoxemia causes sudden mental-status change and apprehension. Syncope, present in about 8-19% of PE patients, signals reduced hemodynamic reserve.

Assess activity intolerance: weakness, fatigue, or dyspnea on exertion. PE survivors walk shorter distances on the 6-minute walk test and score around the 40th percentile on quality-of-life measures, the "post-PE syndrome."

Restore or maintain a patent airway. Use deep breathing, coughing, and suctioning. ABGs in PE typically show hypoxia, hypocapnia, and respiratory alkalosis.

Elevate the head of the bed and assist with frequent position changes. This maximizes chest expansion and aerates different lung segments, improving oxygen diffusion.

Monitor the client frequently and arrange for someone to stay as indicated. A patient who develops PE needs emergency measures immediately.

Help the client manage fear and anxiety. Encourage expression of feelings and normalize the sense of impending doom. Understanding the basis of the fear helps the patient regain control.

Monitor serial ABGs and pulse oximetry. ABGs in PE show hypoxia (PaO₂ <80 mm Hg), hypocapnia, and respiratory alkalosis, though up to 20% of PE patients have normal arterial oxygen pressure and a normal alveolar-arterial gradient.

Prepare for a lung scan and other imaging. Physical exam is not a sensitive indicator. Venous ultrasonography with or without color Doppler is the most common test; MRI and D-dimer also help. For PE, echocardiography may show right ventricular changes. A V/Q scan, spiral CT, MR angiography, and pulmonary arteriogram may be used.

Administer supplemental oxygen by the appropriate method. The PE patient needs oxygen immediately and is at high risk for cardiopulmonary arrest until the clot lyses or reabsorbs.

Administer IV or oral fluids as indicated. Fluids reduce blood hyperviscosity and support circulating volume and perfusion.

Administer medications as prescribed. Unfractionated heparin (IV) or low-molecular-weight heparin (subcutaneous) prevents further clotting. Thrombolytics, when used, should start within the first 24 hours for best results.

Assist with chest physiotherapy: postural drainage and percussion of nonaffected areas, blow bottles, and incentive spirometry. This deepens respiratory effort and drains secretions for clearance by coughing or suctioning.

Prepare for bronchoscopy or surgical intervention if indicated. Bronchoscopy removes clots and clears airways. Surgery for PE includes vena cava ligation or an intracaval umbrella, for patients with recurrent emboli despite anticoagulation, when anticoagulation is contraindicated, or when septic emboli below the renal veins do not respond.

2. Managing Acute Pain

DVT often runs clinically silent but can present with calf pain, edema, and venous distention from inflammation and obstructed venous return. The high-probability complication is PE, which adds chest pain.

Assess pain on a 0-10 scale and palpate the leg cautiously. Pain tracks with the extent of involvement, hypoxia, and edema. Suspect superficial thrombosis in a varicose-vein patient with calf tenderness; pain rises with weight-bearing.

Assess for Homan's sign. Calf pain on dorsiflexion may be positive, but a negative sign does not rule out obstruction, and the test is usually skipped now in favor of objective imaging. Postpartum patients may have a strained muscle from delivery positioning, lowering its value. Assess instead for redness, tenderness, warmth, and increased leg circumference.

Monitor vital signs, noting elevated temperature or pulse. Watch for a slight temperature rise, possibly to 101℉ (38.3℃), and report it. Rising vital signs may reflect increasing pain or the inflammatory process.

Watch for sudden or sharp chest pain, dyspnea, tachycardia, or apprehension. These signal PE, with tachypnea, falling oxygen saturation, and a sudden mental-status change.

Encourage position changes with the extremity elevated. Leg exercises and walking contract the muscles and promote venous return. Elevate the legs above heart level and avoid prolonged sitting or standing.

Elevate the affected extremity and provide a bed cradle. A cradle lifts the bedclothes off the leg, reducing sensitivity and improving circulation.

Maintain bed rest during the acute phase. Bed rest with elevation reduces interstitial swelling and promotes venous return.

Explain procedures, treatments, and interventions. Involving the client increases her sense of control and lowers anxiety. She will need help with personal care while on bed rest; involve the family.

Apply moist heat to the extremity. Check the temperature first to avoid a burn. Use a chemical heat pack or an Aquathermia pad to keep the compress warm.

Apply compression stockings as appropriate. For superficial, mildly tender thrombophlebitis in a varicose vein, elastic support plus analgesics is usually enough. Leaving the leg dangling without support increases swelling and pain.

Administer antipyretics, anti-inflammatory drugs, and analgesics as indicated. Salicylates, indomethacin, and ibuprofen help superficial thrombophlebitis. Salicylates, ibuprofen, and dipyridamole have been used as antithrombotics, though their benefit here is undocumented.

Apply magnesium sulfate (Epsom salt) compresses as indicated. Magnesium sulfate compresses can ease swelling and pain, though surgery is sometimes needed to remove the clot.

3. Reducing Anxiety

Anxiety is a cardiovascular risk factor. Even brief stress drives alpha-adrenergic vasoconstriction that leaks intravascular fluid into the interstitium, raising viscosity and thrombosis risk. Catecholamine-induced hypertension can also rupture atherosclerotic plaque.

Determine the availability of support after discharge and prioritize household tasks. This identifies needs and starts problem-solving before discharge. She will need help with personal care while on bed rest; involve the family if she wishes.

Assess the client's and family's level of understanding. Gauge their grasp of the diagnosis and their ability to cope with an unexpected extended recovery.

Monitor vital signs and behavioral signs: restlessness, irritability, crying. These reflect a change in anxiety and coping. An apprehensive, diaphoretic patient with a sudden mental-status change may be having a PE.

Explain the condition, diagnostic procedures, and treatment. This involves the client and decreases fear of the unknown. Focus on modifiable DVT risk factors and the danger signs to report.

Involve the client and family in the care plan; review instructions and restrictions. Involvement gives a sense of control and clarifies the purpose of restrictions. Teach prevention for future pregnancies: nonconstricting clothing, resting with feet elevated, and daily ambulation.

Help the client care for herself and the infant. Meeting her needs lowers anxiety. She can keep breastfeeding on heparin. If restarting after warfarin, have her manually express milk at feeding times to maintain supply.

Encourage relaxation techniques. Relaxation and breathing exercises reduce muscle fatigue and the oxygen demand that fear and anxiety drive up.

Provide opportunities to verbalize concerns. Expressing concerns lowers tension and helps the client regain a sense of control.

Encourage frequent contact with spouse and children, in person or by phone, and rooming in with the newborn as the condition allows. This reduces isolation and eases the transition home. If the infant is discharged, the family can bring the baby for feedings or the mother can express milk.

Refer to social services, visiting nurse, or home care as appropriate. Home visits often draw blood for clotting studies and help the client cope with therapy.

4. Patient Education and Health Teaching

Teaching the client and family about the condition and treatment builds confidence and supports both the emotional and physical recovery.

Assess the client's knowledge of postpartum thrombophlebitis and correct misconceptions. Baseline understanding clarifies what teaching is needed.

Provide information about management and diagnostic tests. Physical exam is not sensitive for thrombosis. Venous ultrasonography with or without color Doppler is the most common test; MRI and D-dimer also help. Pregnancy limits the usefulness of ABGs and oxygen saturation in diagnosis.

Identify signs requiring provider notification, such as pain, swelling, and tenderness in one leg. DVT signs are often absent or diffuse: calf swelling, erythema, warmth, tenderness, pedal edema. Report sudden chest pain, dyspnea, and tachypnea immediately. Bleeding or worsening needs prompt evaluation.

Demonstrate anti-embolic stockings. Remove elastic stockings briefly at least twice daily. Continuous constriction can reduce surface perfusion and fatigue the muscle. Removal lets you check for further vascular involvement. Fit them correctly to avoid excess pressure.

Explain the purpose of bed rest or activity restrictions and elevate the extremities at rest. Rest lowers tissue oxygen demand and reduces the risk of thrombus fragmentation. Balancing rest with activity prevents exhaustion.

Teach bleeding precautions: electric razor, soft toothbrush, no barefoot walking. Anticoagulation increases bleeding risk; these practices prevent injury.

Discuss the anticoagulant's purpose and dosage. Stress taking it exactly as prescribed. The client usually goes home on oral anticoagulants and needs the schedule and side effects explained. If subcutaneous injections are used, teach administration and site rotation, and confirm good technique before discharge.

Identify drug interactions (antibiotics, NSAIDs, barbiturates, diuretics, vitamins) and stress reading OTC labels. Oral anticoagulant therapy may last 3-4 months. Salicylates and excess alcohol decrease prothrombin activity; vitamin K in multivitamins increases it; antibiotics alter intestinal flora and may interfere with vitamin K synthesis; barbiturates increase coumarin metabolism. Aspirin inhibits clotting-factor synthesis and prolongs clotting time.

Identify anticoagulant effects needing medical attention. Early detection of prolonged clotting time prevents serious complications. Report nosebleeds, gum or mouth bleeding, black tarry stools, coffee-ground vomitus, red to brown speckled mucus, increased lochia, and unexplained bruising.

Stress the importance of medical followup and lab testing. The therapeutic range is narrow and complications can be deadly, so close monitoring of clotting times and dosage is essential.

Educate on contraception during anticoagulant therapy. Use reliable contraception on warfarin because it is teratogenic. Oral contraceptives are contraindicated because of the increased thrombosis risk.

Tell the client that breastfeeding is allowed on anticoagulant therapy. Neither heparin nor warfarin is excreted in significant amounts in breast milk. If the infant is discharged, the family can bring the baby for feedings or the client can express milk to send home.

5. Medications and Pharmacologic Support

Anticoagulants such as heparin or low-molecular-weight heparin prevent clot progression, inhibit new clot formation, and promote dissolution of existing clots. Monitor the response with regular coagulation studies to keep dosing therapeutic.

Antimicrobial agents. If infection is the underlying cause, an antibiotic is prescribed. Reproductive-tract infection can cause septic pelvic thrombophlebitis. Some clinicians stop antibiotics after clinical improvement (afebrile at least 48-72 hours) and normalized labs; others continue until discharge.

Thrombolytic agents (streptokinase, urokinase). Used for acute or massive DVT to prevent valvular damage and chronic venous insufficiency. Heparin usually starts several hours after thrombolytic therapy completes.

Heparin (continuous IV, intermittent heparin lock, or subcutaneous) or coumarin derivatives. Restart heparin 12 hours after cesarean delivery or 6 hours after vaginal birth, assuming no significant bleeding. Heparin is preferred initially for its prompt, predictable action against thrombin and because its large molecular size keeps it out of breast milk. ACOG names heparin compounds as the preferred anticoagulants in pregnancy and the puerperium because they do not cross the placenta, and it recommends low-molecular-weight heparin over unfractionated heparin for most patients because of its greater reliability and ease of administration. Coumadin, which blocks prothrombin formation from vitamin K, may be used for long-term therapy after discharge.

What is postpartum thrombophlebitis? Thrombophlebitis is inflammation of a vein with clot formation. After birth the patient sits in a hypercoagulable state, because pregnancy raises fibrinogen and clotting factors while lowering clot-dissolving factors. Three conditions dominate this period: superficial venous thrombosis, deep vein thrombosis (DVT), and pulmonary embolism (PE).

Why are postpartum patients at higher risk for blood clots? Pregnancy and the postpartum period carry a fourfold to fivefold increased risk of venous thromboembolism compared with nonpregnant women (ACOG), driven by venous stasis from the heavy uterus, the hypercoagulable state, and vessel injury. What are the warning signs of pulmonary embolism? Sudden chest pain, shortness of breath, cough, tachycardia over 100 beats per minute, tachypnea over 20 breaths per minute, apprehension, diaphoresis, and a sudden change in mental status from hypoxemia. PE is an emergency, so report any sudden breathing difficulty in a patient with DVT immediately and give oxygen.

What anticoagulant is preferred in pregnancy and postpartum? Heparin compounds are preferred because they do not cross the placenta, and ACOG recommends low-molecular-weight heparin over unfractionated heparin for most patients because it is more reliable and easier to dose. Warfarin is reserved for after delivery and requires reliable contraception because it is teratogenic.

Can a patient breastfeed while on anticoagulation? Yes. Neither heparin nor warfarin passes into breast milk in significant amounts, so breastfeeding can continue on anticoagulant therapy. If feeds are interrupted, the family can bring the infant for feedings or the patient can express milk to maintain supply.
