Steps to become a telemetry nurse

Find the right school. Look for accreditation, since graduates of non-accredited programs may not qualify to take the NCLEX, which all 50 states require for licensure. Check job placement numbers and career counseling, and consider online options for coursework, remembering that hands-on training still happens in person.

Choose your degree. Earn an ADN or BSN. Telemetry nurses work mainly in hospitals, which increasingly require a BSN.

Get into a program. Prerequisites often include a high school diploma or GED with a minimum GPA, prior biology and chemistry coursework, and minimum SAT scores where required.

Complete coursework and clinicals. A BSN takes about four years of full-time study. Coursework may include biology, chemistry, anatomy, ethics, physiology, nursing practice, and healthcare policy. Most programs require 700 to 800 clinical hours. If you want telemetry, complete as many of those hours as you can on a telemetry unit. Another way to build experience is to work as a patient care tech, patient care associate, or centralized monitor technician before becoming an RN, and to take any telemetry courses your program offers.

Pass the NCLEX-RN. After graduating, you must pass the National Council Licensure Examination-RN, overseen by the National Council of State Boards of Nursing (NCSBN), in the state where you plan to practice.

Meet state requirements. Once you pass, apply for your RN license through your state board. Most boards require proof of a nursing degree, and some ask for fingerprints or a background check.

Land your first position. New grads often need general experience and training before a telemetry role. Becoming ACLS certified is one of the best ways to market yourself. Advanced Cardiovascular Life Support builds on CPR and basic life support with deeper cardiovascular knowledge. Many employers pay for the course, but arriving pre-certified shows commitment and skill with heart rhythms and interventions.

Pursue certification. Advanced certifications are optional but pay off, making you more valuable and more marketable for role transitions. Most require one to two years of experience and clinical hours, and many hospitals reward them with higher salaries and employer-paid continuing education.

Certification options

Three popular certifications for telemetry nurses:

  • Progressive Care Certified Nurse (PCCN): from the American Association of Critical-Care Nurses (AACN), for excellence in caring for acutely ill adult patients.
  • Tele-ICU Adult Acute/Critical Care Nursing Certification (CCRN-E): also from the AACN, for nurses who monitor patients remotely in tele-ICUs.
  • Cardiovascular Nursing Level I (CVRN-BC Level I): from the American Board of Cardiovascular Medicine, for nurses in telemetry, progressive care, pediatric intensive care, and interventional cardiology.

Prep and exams. The AACN offers free and paid prep for the CCRN-related exams; the CVRN-BC has its own resources. The PCCN and CCRN-E exams are online, with three hours to answer 125 questions on clinical judgment and ethical care. The CVRN-BC Level I exam is online with three hours for 150 multiple-choice questions on bedside assessment, EKG concepts, heart failure, and pacemaker management.

Maintenance. The PCCN and CCRN-E last three years and renew with 100 continuing education credits or by retaking the exam. The CVRN-BC also lasts three years and renews with 30 continuing education credits. Confirm current requirements with the certifying body before applying.

Where telemetry can lead

Several specialties are a natural next step and can be even more intense:

  • ER nurse: treats trauma and injuries needing immediate care; after two years of emergency experience, you can certify through the Board of Certification for Emergency Nursing (BCEN).
  • ICU nurse: provides around-the-clock care for life-threatening conditions, with AACN certifications in adult, pediatric, and neonatal nursing.
  • Flight nurse: provides care during medical air transport, with an advanced BCEN certification.

What the work involves

Technology is at the core of the job. Telemetry nurses use electrocardiograms (ECGs) to monitor the heart's electrical activity, preparing patients, taking the reading, and interpreting results. They also track blood pressure, oxygen saturation, and respiration. Beyond standard duties like consulting with physicians and families, telemetry nurses:

  • Watch for arrhythmias and other heart abnormalities
  • Respond to and stabilize heart irregularities, including false alarms
  • Intervene with defibrillation
  • Perform stress tests
  • Assist with cardioversions to restore normal heart rhythm

Monitoring can happen at the bedside, through a remote system where alarms and readings occur away from the patient, or both.

What it takes

The role rewards strong critical thinking in a data-heavy field, the ability to multitask across many patients, adaptability when processes or treatments change, comfort with specialized technology, and a willingness to keep learning as the field evolves.

How long it takes

Plan on about five years if you complete a bachelor's degree and gain experience before your first telemetry position. Most institutions also require nurses to pass a cardiac rhythms and interventions test before treating telemetry patients.

Pros and cons

Pros: continued learning in cardiac care, job security, and the reward of helping people at a vulnerable time.

Cons: high workload and nurse-to-patient ratios, high stress, and night and overnight shifts.

One piece of advice from the field: prioritize and delegate effectively, lean on teamwork, and when interviewing, ask about nurse-to-patient ratios and unit size.

Salary and job outlook

The BLS reports a median annual RN salary of $93,600 (2024). It doesn't break salary out by specialty, but BSN-holding nurses who specialize can earn more. Pay also varies with experience, certification, and location.

The BLS projects RN employment to grow about 5% from 2024 to 2034, faster than the average for all occupations, with roughly 189,100 openings per year. Much of that demand comes from the large, aging baby boomer population, which typically needs more care for conditions like diabetes and heart disease.
