Disease Spotlight: Sinusitis
Sinusitis is inflammation of the epithelial lining of the sinus cavities. The swelling presses against bone that cannot stretch, which causes severe pain and blocks the sinus passage. The real danger is an untreated infection: microorganisms can travel up the sinus passages into brain tissue. Symptoms include pain, swelling, and tenderness around the cheek, eyes, or forehead, a blocked nose, reduced sense of smell, a sinus headache, and a high temperature of 38°C.
What Decongestants Do
Decongestants cut the overproduction of secretions by causing local vasoconstriction in the upper respiratory tract. That shrinks swollen mucous membranes, opens clogged nasal passages, and promotes drainage and airflow. The three classes are topical nasal decongestants, oral decongestants, and topical steroid nasal decongestants.
Therapeutic Actions
Topical nasal decongestants imitate the sympathetic nervous system to cause vasoconstriction, decreasing edema and inflammation of the nasal membranes. They relieve the congestion of the common cold, sinusitis, and allergic rhinitis, and dilate the nares to ease medical examination or relieve the pain and congestion of otitis media.
Oral decongestants decrease nasal congestion from the common cold, sinusitis, and allergic rhinitis. They shrink the nasal mucous membrane by stimulating alpha-adrenergic receptors, promoting sinus drainage and airflow.
Topical nasal steroid decongestants work by an unknown mechanism. Their anti-inflammatory action comes from a direct local effect that blocks many of the reactions behind the inflammatory response.
Indications
Topical nasal decongestants relieve the discomfort of nasal congestion from the common cold, sinusitis, and allergic rhinitis, and relieve the pressure of otitis media. Oral decongestants decrease nasal congestion from the common cold and allergic rhinitis and relieve the pain and congestion of otitis media. Topical nasal steroid decongestants treat seasonal allergic rhinitis when other preparations have failed and relieve inflammation after removal of nasal polyps.
Pharmacokinetics
Topical nasal decongestants act almost immediately because they are applied directly, with less chance of systemic effects. Pseudoephedrine, an oral agent, is well absorbed and peaks quickly, in 20 to 45 minutes. Topical nasal steroids are not immediate and may take up to 1 week to produce any change.
Contraindications and Cautions
Use caution when a lesion or erosion in the mucous membrane could allow systemic absorption, and in any condition that sympathetic activity might worsen. Use caution in pregnancy and lactation. Use caution in any patient with an active infection, including tuberculosis, because systemic absorption would interfere with the inflammatory and immune response.
Adverse Effects
- Rebound congestion. Frequent or prolonged use of a topical decongestant causes rebound congestion, technically rhinitis medicamentosa. It can set in after only a few days of use, which is why these sprays should be limited to about three days.
- Topical nasal decongestants. Local stinging and burning, mostly the first few times the drug is used.
- Oral decongestants. Because the drug is systemic, sympathomimetic effects are more likely: anxiety, tenseness, restlessness, tremors, hypertension, arrhythmia, sweating, and pallor.
- Topical nasal steroid decongestants. Local burning, stinging, dryness of the mucosa, and headache.
Interactions
- Cyclopropane or halothane. Topical nasal decongestants are contraindicated with cyclopropane or halothane anesthesia because serious cardiovascular effects could occur.
- OTC products. Many cold, allergy, and flu remedies contain pseudoephedrine; combining them can cause serious adverse effects.
- Acute infections. Nasal steroids block the inflammatory response, so they are contraindicated in acute infections such as Candida albicans, tuberculosis, and airborne infections (chicken pox, measles), because systemic absorption would interfere with the inflammatory and immune responses.

Screen for cautions and contraindications: allergy to the drug or its vehicle, glaucoma, hypertension, diabetes, thyroid disease, coronary disease, prostate problems, pregnancy and lactation, and acute infections, all of which the sympathomimetic effects could worsen. Establish baseline data with a physical exam. Check skin color and temperature for the sympathetic response, orientation and reflexes for CNS effects, and pulse, blood pressure, and cardiac auscultation for cardiovascular effects. Evaluate respirations and adventitious breath sounds. Inspect the nasal mucous membrane for lesions that could allow systemic absorption and to gauge the decongestant effect. Monitor urinary output for retention and temperature for acute infection.

Nursing Implementation with Rationale
Teach proper administration: clear the nasal passages first, tilt the head back when applying drops or spray, and keep it back for a few seconds afterward so the drug contacts the affected membranes instead of trickling down the throat and causing systemic effects. Caution the patient not to use the drug longer than 5 days and to seek care if symptoms persist past that, which helps catch underlying conditions. Warn that these drugs hide in many OTC products, so combining preparations with the same ingredients risks overdose. Provide safety measures if dizziness or sedation occurs, and comfort measures for congestion (humidifier, increased fluid intake, cool environment, avoiding smoke). For topical nasal steroids, tell the patient to keep using the drug regularly even without immediate results, because benefits may take 2 to 3 weeks. Monitor for acute infection that needs medical intervention, and counsel the patient to avoid places with airborne infection because steroid use blunts immune and inflammatory responses. Provide thorough teaching on drug name, dosage, adverse effects, warning signs, and the need for monitoring.
